In May 2015, the Bellefontaine Police Department launched the Team Effort Program, created with the best interests of you and your loved ones in mind. This program is designed to help us assist you quickly if your loved one goes missing.

The program allows you to provide crucial details about your family member to your local police department, enabling us to respond swiftly in case of an emergency. We recognize that the distress of a missing loved one is amplified when their condition makes it more likely for them to become lost. By proactively sharing this information, you can ensure that we are better prepared to help when every second counts.

By completing and submitting the form below, you allow us to update our department records and flag your loved one's address in our system. This ensures that, should your family member go missing, we’ll have immediate access to vital information about their condition, which could help us locate them more quickly or provide the necessary medical attention if needed.
